In the world of alternative payment models, bundled payment has quickly taken off. Under the BPCI program, conveners were able to voluntarily take on risk in bundled payment pilots. Now, in 2016, CMS is mandating bundled payment participation for the first time. As health care continues to move in the direction of alternative payment models, it is critical that hospitals, health systems, physician groups, post-acute care, and health plans begin to build and implement their bundled payment strategies.
In its second year, the Bundled Payment Summit takes an in depth look at redesigning care, establishing post-acute care partnerships, and engaging physicians in this payment and care coordination strategy. Here, attendees learn best practices in building and implementing an effective bundled payment strategy from their peers who are achieving success with this alternative payment model. Key topics of discussion include:
- CMS address
- Hearing results and best practices following the launch of the CJR model, and what to expect next in 2017
- Building bundled payment programs as a clinical care model redesign program
- Leveraging Care Managers and Population Health Coaches to achieve bundled payment success
- Strategies for tracking patients across the continuum of care
- Understanding how bundled payment can offer a solution to fragmentation, one of health care’s biggest challenges
- Exploring tactics for receiving real time data
- Discovering patient engagement strategies for pre-op patients
- Physician engagement strategies
- Gainsharing and risk sharing
- Care management across the continuum of care
- Post-Acute Care engagement and scorecards
- Appropriateness of care versus utilization
- Innovative case studies in bundled payment models
