Care Coordination: How Physicians and Health Support Organizations Can Collaborate to Improve Patient Outcomes
Improving the coordination of patient care across the distinct and often rigid silos of the U.S. health care system is essential to improving both health care quality and health outcomes. Traditionally, care coordination has been the responsibility of primary care physicians. However, the increasingly demanding requirements for care coordination (especially for patients with chronic conditions) are taxing the capacity of many primary care practices as they are organized and managed today. To the frustration of physicians, and their patients, it is increasingly difficult to marshal resources, direct services, and effectively share essential information in a coordinated manner in order to support the fulfillment of best-practice care plans. To address this burgeoning challenge in health care, Johns Hopkins and American Healthways brought together more than 200 practicing physicians, thought leaders and subject matter experts from across the country to develop a consensus set of recommendations on how to improve patient care through better coordination between physicians and health support organizations. Dr. Jim Pope, Chief Medical Officer of American Healthways, will present key findings from this summit on behalf of its participants and engage attendees of the World Healthcare Congress in sharing additional insight on this critical topic.
