The Current Landscape of Disease Management and P4P:
Linking Transparency, Incentives, and IT |
2:15 pm - 3:00 pm |
Beyond P4P: What it took to Improve Care and Reduce Costs in a Primary Care Group |
Quality incentives are necessary, but insufficient. Working in partnership with a 12-physician group and Healthways, HealthSpring found a way to align data, dollars, and nursing care support within the day-to-day work-flow of Sumner Medical Group. The result was an unprecedented increase in clinical quality resulting in a significant decrease in avoidable ER visits and hospital admissions.
- Learn the practical realities of engaging MD groups in a P4P partnership.
- Learn the practical realities of creating fair, robust outcomes metrics using the medical record (not claims) as the basis for measurement and payment.
- Learn the practical realities of re-designing practice workflows to fully leverage physician extenders to reduce practice variability and increase quality of care...while reducing the demands on the MD’s time.
- Learn how health plans, physician practices and health improvement organizations can effectively integrate efforts to enhance coordination of patient care.
