John has over 20 years of consulting, business development, operations, and management experience working with technology and health care companies. He has recently led RPM Program Design projects with the Mayo Clinic, Riverside Health System, Renown Health System, and Ascension Health. Prior to co-founding Trapollo, he was the General Manager of GTSI’s Health care Business Unit which provided information technology solutions to health care focused government agencies. Mr. Aldridge has a Bachelor of Science, Business Administration & Marketing from the University of Maryland and an MBA from George Washington University.

Dr. Atreja has received formal training in public health and is board certified in gastroenterology, clinical informatics and internal medicine. Over the last fifteen years, he has led many public health and informatics initiatives at Cleveland Clinic and Mount Sinai Medical Center, NY that includes developing online education modules, leading EHR implementation, performing analytics on healthcare data and developing enterprise-wide mobile apps. As Chief Technology Innovation and Engagement Officer, Medicine, he leads the Sinai AppLab (www.sinaiapplab.org) that is one of the first collaborative hub within academic medical center to build and test disruptive mhealth technologies.

Dr. Atreja leads scientific registries for American Gastroenterology Association and serves in Innovation Advisory Board for American College of Cardiology. As an intrapreneur, Dr. Atreja has won innovation awards at Cleveland Clinic and Mount Sinai, successfully licensed technologies from academic centers and advises startups, accelerators and Fortune 500 companies in digital medicine. Recently, Dr. Atreja established Network of Digital Medicine (www.nodehealth.org) to connect innovation centers worldwide and share best practices for digital medicine innovation and implementation between industry, payers and health systems. Dr. Atreja serves as Scientific Co-founder for Mount Sinai Spinoff, Responsive Health (http://responsivehealth.org) that aims to bring first enterprise-wide app curation, prescription and engagement platform to risk sharing hospitals and payers. Dr. Atreja has published more than 60 papers and has been a keynote speaker globally on topics related to digital medicine and health system transformation.

Davis Baird is a Senior Program Associate with the Aging and Disability Business Institute (the “Business Institute”) at the National Association of Area Agencies on Aging (n4a). In this role, Davis provides strategic programmatic and policy support to advance the mission and work of the Business Institute to connect community-based organizations (CBOs) and health care payers and providers in their efforts to deliver on the promise of robust population health and value-based care. Davis is responsible for the Business Institute’s online presence, assists with partner and stakeholder outreach and engagement, furnishes technical assistance to field-based providers, and manages an interactive learning collaborative of operationally advanced CBOs.

Prior to joining n4a, Davis served as a Research Associate at the Altarum Institute, a large nonprofit health systems research and consulting organization in Washington DC. At Altarum, Davis worked to build an innovative grassroots policy and advocacy movement around family caregiving and its impact on the health care and community supports systems. Before his time at Altarum, Davis worked as a volunteer advocate at an Area Agency on Aging in Arlington, VA, as well as a Research Assistant on a special two-year project at a Continuing Care Retirement Community in Charleston, SC.

Davis received his bachelor’s degree in Psychology at The College of Charleston, and his Masters’ degree in Gerontology from the University of Southern California, graduating with honors at both institutions.

Prior to her present role as Director of Public Payer Programs and Policy, Cecilia served in Massachusetts Governor Deval Patrick’s administration, as Deputy Director of Policy and Cabinet Affairs when the state’s health reform initiatives were being implemented. She was then promoted to Deputy Legal Counsel and worked on complex issues impacting the Governor’s Office and the Commonwealth’s health and human services agencies.

Cecilia’s passion for health care policy started during her time as a Legal Fellow at the Massachusetts Executive Office of Health and Human Services where her work focused on legal matters related to state’s income eligible healthcare program.

Deeply committed to serving her community, Cecilia currently sits on the Board of Directors for the Massachusetts Children’s Alliance. Previously, she has served as the Senate Captain for the Early Education for All Campaign, as a Member of Rappaport Center for Law and Public Policy Advisory Committee, and as a Board Member of the Disability Law Center of Massachusetts.

Debby is the Sr. Director of Regulatory Incentive Programs at Banner Health. Debby previously worked as the Director of Nursing serving the Banner Boswell Medical Center Inpatient Rehabilitation Unit and Skilled Nursing Facility, in Sun City, Arizona. She trained in New Jersey for her nursing degree in the early 90’s, and went on to earn her Bachelor’s degree in Nursing from Grand Canyon University, after relocating from New Jersey in 2004. Debby worked for many years as a bedside nurse in numerous levels of care, but found her true passion in the Intensive Care Unit. She continued her education and obtained a Master’s of Science in Nursing from Grand Canyon University, and National Certification in Critical Care nursing.

Today she oversees a department of 7, responsible for the Meaningful Use program for 28 Inpatient facilities ranging from CAH to Research Universities. She also manages the MU program for over 3,000 employed providers. Recently, she has added responsibility for Banner’s MACRA strategy, which includes participation in 2 MSSP programs, CPC+ and OCM-1 sided risk.

Deborah Boswell is the Director of the Harris Health System Community Outreach Services department. Mrs. Boswell is positioned to implement evidence based health promotion interventions outside the walls of the safety net organization to reduce health disparities including barriers to accessing preventive and primary healthcare services. She deploys outreach teams of multilingual health educators and community health workers to the most vulnerable communities of uninsured or underinsured residents to provide health education and assistance with resource applications; including the Harris Health Financial Assistance program. Along with her team, Mrs. Boswell works interdepartmentally to promote wellness, prevention and early detection services across the Ambulatory Care Services platform.

Mrs. Boswell successfully facilitates and participates with grant writing teams resulting in various awards from public, private, state, and federal funding sources that enhance patient care services. She is the program manager for the OB Navigation Program which is funded through the Medicaid 1115 Waiver Delivery System Reform Incentive Payment (DSRIP) program. This program is implemented in 8 OB/Primary Care centers by Registered Nurse Navigators and community health workers who assist pregnant women through delivery and postpartum to reduce their risk of preterm birth. To date, nurse navigators have enrolled 3,184. Data supports higher retention in prenatal care achieved as well as greater numbers completing the postpartum visit than non-navigated patients. She is the program manager for the Strong Start Initiative for Mothers and Newborns, a 4 year funded award by the Center for Medicare and Medicaid Services (CMS), in which Licensed Master Social Workers and community health workers provide social support for our prenatal patients enrolled in CenteringPregnancy, evidenced based group prenatal care. This program has successfully completed implementation in 6 Primary Care centers and currently in the final year of evaluation. Increased patient satisfaction, delivery retention, and navigation into interconception care have been positive outcomes. The Texas Health and Human Services Commission and Department of State Health Services have been tasked by our State legislature to provide contract funding for women’s health services. Mrs. Boswell had oversight of the Expanded Primary Healthcare services for women, implemented in 2013, providing for contraceptive services. She successfully worked with her team to develop and receive contracts for the new State programs, Healthy Texas Women and Family Planning Programs focused on reproductive health and contraceptive choices for family planning currently being implemented. As program manager, her responsibility also includes evaluation of process and impact outcomes, corrective action planning and quality assurance reporting.

Mrs. Boswell represents the Harris Health System in a number of community partnerships and coalitions impacting the residents of Harris County. She has worked collaboratively with other City, County, and non-profit community based organizations to improve population health. She is an active stakeholder on the Enroll Gulf Coast Collaborative promoting access to healthcare coverage through the ACA Marketplace. She was the team member for Harris Health, along with a City of Houston and Gateway to Care, competitively awarded by the National League of Cities Institute for Youth, Education and Families to mobilize communities to expand Medicaid and CHIP enrollment. Additional partnerships and collaborative groups include the IMPACT Collaborative, the Women’s Mental Health Task Force, Gateway to Care Collaborative, Texas Collaborative for Healthy Mothers and Babies.

Mrs. Boswell has held her current position as Director since the inception of the outreach department in 1999, but initially joined Harris Health System Health Promotion Services as a Health Educator in 1996. Prior to her work at Harris Health, she worked in Employee Wellness after receiving her Bachelor of Science degree in Kinesiology from Texas A&M; University.

Michelle Brown has served as an innovation leader and key decision maker for the Choice Health Care Network (CHN) since 2005 and the Morehouse Choice Accountable Care Organization and Education System since its formation in 2012. Both organizations collaboratively operate as safety net health care delivery networks, integrating federally qualified health centers, and other community-based agencies, partners and medical practices, that render over 500,000 patient encounters per year in metropolitan Atlanta. Brown led the development and implementation of the Network’s advanced and NCQA-recognized patient-centered medical homes and the ACO’s application to participate in the Medicare Shared Savings Program that was granted by CMS in 2013.

Entering her 26th year of progressive health care administration and managed care experience, Brown has proven expertise in driving integrated deliver models to consistent success through affordable coordinated care, health delivery re-engineering, quality outcomes and value-based contracts that have generated return on investment. She has toned specialization in organic growth initiatives, health program planning and evaluation, strategic and tactical management and provider/payer contract administration. Brown’s experience is largely derived from prior leadership roles in building and managing health care provider networks for leading Fortune 100 health insurance companies, independent practice associations and hospital systems. Brown also has notable achievement in forging enterprise-wide health information technology solutions that bridge the gap between innovation and action for new health care business development. She has helped the Networks’ Medicaid and Medicare populations that are under incentive-based contracts grow by over 30,000.

Known for her unyielding passion to improve access and cost for health equity, Brown serves numerous partnership and stakeholder organizations, including the Atlanta Civic Site System of Care Healthy Beginnings Advisory Board; the Georgia Department of Community Health-CMO Quality Medical Management Committee, AID Atlanta, the Atlanta Regional Collaborative for Health Improvement; and has worked with the HIV in Primary Care Learning Community, HealthHIV and National Center for HIV Care in Minority Communities as a Practice Transformational Lead. She is a member of the American Public Health Association, the Healthcare Financial Management Association, the National Association of Community Health Centers, and the American College of Health Care Executives. Brown holds a Bachelor’s degree in business administration and a Master’s degree in Public Health. She is also a registered yoga instructor.

Dr. Church is a private practice rural family physician in Hiawassee, GA where he lives on a small farm with his wife and four children, working in the office, ER, and hospital. He earned his medical degree from East Carolina School of Medicine and completed residency training at AnMed Health. Dr. Church is an active member of the American Academy of Family Medicine and the Georgia Academy of Family Physicians, serving on numerous state and national committees and work groups, with an emphasis on chronic care management and quality improvement strategies; Church serves as an Alternate Advisor to the AMA CPT Editorial Panel. He is recognized by AAPC as a Certified Professional Coder and Risk Adjustment Coder. In addition, he serves as adjunct faculty in Family Medicine for the Medical College of Georgia, regularly precepting 3rd year medical students who help keep his passion for medicine alive. He has recently been recognized as Georgia Family Physician of the Year.

Donna Cohen serves as the Deputy Director for Population Health at Alliant Quality. Her career in health care began more than 30 years ago and consists of experience in a variety of clinical settings. She is a Registered Nurse and a Certified Case Manager. Her experience ranges from the inpatient hospital setting, to physician practice management as well as Managed Care where she was the director of Case Management and Utilization Review. Before joining Alliant Quality, she was a practice manager for a large physician practice group and with the implementation of EPIC, she led the clinical informatics team for the physician practice group of 150 clinicians.

Julia has dedicated her career to advancing health care. She is passionate about enabling transformation in pursuit of the Triple Aim. Prior to joining Clarify Health, Julia served in multiple roles at Aetna, most recently as SVP, Strategy & Operations, Accountable Care Solutions, where she was responsible for the development, growth, and performance of Accountable Care Organizations (ACOs) and ACO-based health plans. Prior to that, Julia was a Project Leader at The Boston Consulting Group (BCG), and held product and commercial roles at The Advisory Board Company, a health technology & services company. Julia holds an MBA from The Wharton School, University of Pennsylvania, where she was a Howard E. Mitchell Fellow, and a BA in Government and Psychological & Brain Sciences from Dartmouth College.

Dr. Damani founded MD Revolution with the purpose of building the gold standard technology enabled service platform for chronic care management (CCM). Today, MDR is the leading platform in the nation for Medicare’s CCM program and is being leveraged by thousands of patients and providers across the nation. MD Revolution integrates patient generated data, electronic health records, and physician workflow for the purpose of optimizing population health management. He has served in multiple strategic and operational roles, as well as led efforts in raising over $43 million for the founding vision as CEO. Dr. Damani remains highly active as an MD Revolution Board Member. He also serves as a Clinical Instructor in the Department of Family & Preventive Medicine for the UC San Diego Medical School and is a board-certified practicing cardiologist at Scripps Clinic – a top 20 US News and World Report heart care center. Dr. Damani has also served as a scientific adviser for the prominent journal, Science Translational Medicine, and continues to serve as a reviewer for Nature Genetics, Annals of Internal Medicine, American Journal of Cardiology, and the Journal of the American College of Cardiology. Jean Drouin, MD, is the CEO and founder of Clarify Health Solutions, which enables health systems to deliver more satisfying and affordable care through an AI-enabled real-time care guidance platform. Jean is passionate about transforming healthcare delivery across the continuum of care. He is an investor and advisor to several healthcare startups which use technology to improve care. Jean has written and spoken extensively on value-based care, new payment models and the role of big data and analytics in delivering better outcomes. He was formerly a Senior Partner at McKinsey & Company, where his roles included leading the global Healthcare IT and Digital Practice, setting up the UK and Australian Healthcare Practices and serving as the founding Head of McKinsey Advanced Healthcare Analytics (MAHA). He also served as Head of Strategy for National Health Service (NHS) London, which oversaw London’s hospitals and primary care services. Marianne is a Health Information Technology Specialist for the Quality Payment Program at Alliant Quality, and provides technical assistance to large and small practices. She is a Patient-Centered Medical Home certified content expert and an external reviewer for NCQA. Before joining Alliant Quality, Marianne served as the Assistant Director of the Practicum in Primary Care at Appalachian State and supervised Healthcare Management students and rural practice teams achieve PCMH recognition. She is a Certified Professional in Healthcare Quality and has designed hands-on quality improvement learning labs, teaching practicum students about PDSA cycles and Lean healthcare. Marianne has over 20 years experience in healthcare administration, including Medicaid, Medicare and mental health and developmental disability programs.

Neil Gomes (B.Sc., MBA/MMS, M.Ed) is the Senior Vice President for Technology Innovation and Consumer Experience and Chief Digital Officer at Thomas Jefferson University and Jefferson Health System. Neil has worked for the Fortune 500 Tata Group of Companies where he played a leadership role in building the intrapreneurial startup, Tata Interactive Systems, from 60 employees to the world’s largest custom e-learning development firm with 650+ employees in less than two years. Neil left the Tata Group to complete his M.Ed. in Instructional Design at the University of South Florida (USF) whilst progressively working towards the position of Director of eTeaching and Technology and then the Director of Instructional Design and Training at USF Health. While at USF, Neil’s leadership and entrepreneurial acumen helped grow a strategic team of application developers, instructional and multimedia designers, and project managers that generated over $1.5 million in annual auxiliary revenue from research and external development projects while growing online student enrollment from approx. 200 enrollments in 2002 to 60,000+ enrollments a semester by 2012. While at USF, Neil also began working toward his Ph.D., is currently a Ph.D. Candidate (ABD), has authored research articles, a book chapter, and delivered several formal research presentations. At Jefferson, Neil drives digital innovation in healthcare, consumer experience and engagement, training, and education via teams of application and web/mobile developers, portal solutions developers, simulation and UI/UX designers, trainers, documentation and digital consumer experience specialists, instructional designers, e-learning developers, and IT support specialists. Neil also helps define innovation strategy and design innovation development programs via Jefferson’s Innovation Team. Recently, he helped secure a $15+ million donor grant from the Bernie Marcus Foundation to develop a high-tech, consumer-centric, integrative health center at Jefferson and also launched a pioneering collaboration with the IBM Watson IoT team on developing "cognitive concierges" for hospitals. Neil serves as Associate Editor of the Journal for Healthcare Transformation and is a contributor toward the book: We CAN fix Healthcare, the Future is NOW. Neil is also a speaker, agile aficionado, and digital innovation evangelist.

Todd Gottula is Co-Founder and CTO of San Francisco based technology company, Clarify Health. He fundamentally believes that through the deployment of technology and expertise, we can improve the lives of patients and those that care for them. Prior to founding Clarify Health, Todd was the EVP and CTO at Advent Software, a $3B public company, where he led cutting-edge development efforts from ideation through revenue generation, serving as the critical bridge between business and engineering. Todd holds a BS in Chemical Engineering from the California Institute of Technology.

Prior to joining MintHealth, Jerry held senior technology and product software development leadership roles at Amgen, McKesson, Bank of America, JP Morgan and Intuit. Throughout his career, he has developed and launched industry leading digital products in the Health Care, Financial Services and Technology industries. As Senior Strategy Advisor, Jerry advises MintHealth on all aspects of the business and operating models, product development & management as well as platform architecture and digital technologies. Patrick Hammond joined Emory Healthcare in 1991 as an Administrative Resident. He is now a member of the Emory Healthcare Executive Leadership Team.

Stacey Hodgman, RN has been an RN for over 25 years with 18 years of case management expertise and experience working for managed care organizations, short and long-term acute hospitals and worksite wellness programs. She is currently employed with Kindred Healthcare as the Division Vice President of Clinical Integration & Care Navigation.

Ms. Hodgman is responsible for designing and implementing new models of care coordination designed to improve care management and care transitions across settings that seek to improve patient outcomes and experience while transitioning patients efficiently and seamlessly through the post acute continuum. Stacey earned her nursing and undergraduate degrees from Rivier University and her graduate degree from George Washington University. She is board certified in Case Management as well as Certified as a Case Management Administrator. She served two terms on the Board of Directors for the Case Management Society of New England as well as served on the Case Management Advisory Board for the Health Intelligence Network, the Editorial Board for Dorland Health Care Coordination Journal and as a Case Management content expert reviewer for the American Nurses Association Credentialing Center.

Stacey has published several articles in Case Management journals as well as provided numerous CEU presentations on topics such as the value of post acute care and the importance of episodic care management.

Tanvir Hussain, MD, MSc, MHS, is an internist, researcher, and quality executive, whose interests converge upon health systems innovations in population health and quality. In his position as Medical Director of Quality & Analytics for Nebraska Medicine, he serves as physician champion for enterprise quality initiatives touching regulatory programs, technology, delivery redesign, and physician education and engagement. Clinically, in addition to supervising residents, he provides primary care to a high-risk patient panel. He publishes on system redesign and population health interventions, and provides health policy and quality expertise to several national organizations. Prior to his current position, he was a U.S. Fulbright fellow; trained in internal medicine at Harvard’s Brigham & Women’s Hospital; and completed clinical research, quality leadership, and analytics fellowships at Johns Hopkins.

Wright L. Lassiter III is the President and CEO of Henry Ford Health System, overseeing the $5.5 billion health system comprised of six hospitals, a health plan and a wide range of ambulatory and retail health services consisting of more than 60 clinical locations and 28,000 employees.

Lassiter joined Henry Ford in December 2014 as president. In his first year, he led the Board and senior management team through a comprehensive strategic planning effort to position Henry Ford for the future. In addition, he oversaw two successful mergers, one with Allegiance Health in Jackson, Michigan, and the other with HealthPlus of Michigan, a health plan based in Flint, expanding the geographic footprint of the System and generating an additional $1 billion in revenue.

A seasoned health care executive, Lassiter has more than 25 years of experience working in large, complex health systems, including Methodist Health System in Dallas and JPS Health Network in Fort Worth, Texas. Prior to joining Henry Ford, Lassiter was CEO of Alameda Health System in Oakland, California, where he was credited with leading the expansion and turnaround of the $865 million public health system, achieving eight years of positive financial performance, The Joint Commission Top Performer status, significant increases in patient engagement and reductions in patient harm.

Lassiter’s work has received many national accolades, most notably “100 Most Influential People in Healthcare in 2017/2016” by Modern Healthcare; 2016/2012 “Top 25 Minority Healthcare Executives in U.S. Healthcare” by Modern Healthcare; 2014 “Top Blacks in U.S. Healthcare” by the Johns Hopkins Center for Health Disparities Solutions; and 2011 “20 People Who Make Healthcare Better” in the U.S by HealthLeaders. Fast Company prominently featured Lassiter’s work to rebuild Alameda.

An avid community leader, Lassiter has held volunteer positions at the United Way, Salvation Army and YMCA. Currently, he serves on the boards of the American Hospital Association, America’s Essential Hospitals, Federal Reserve Bank of Chicago – Detroit Branch, Detroit Regional Chamber, LeMoyne College, City Year Detroit, and YMCA-USA National Board of Directors.

Lassiter received his master’s degree in Healthcare Administration from Indiana University where he graduated number one in his class, and completed his bachelor’s degree with honors in Chemistry from LeMoyne College in Syracuse.

Dr. Jonathan Lewin was appointed the Executive Vice President for Health Affairs, Emory University; Executive Director, Woodruff Health Sciences Center; and the President, CEO and Chairman of the Board, Emory Healthcare, in February 2016. Prior to his Emory appointment, Dr. Lewin served as the Senior Vice President for Integrated Healthcare Delivery and Co-Chair for Strategic Planning for Johns Hopkins Medicine, as well as the Martin Donner Professor and Chairman of the Russell H. Morgan Department of Radiology and Radiological Science at Johns Hopkins University and the Radiologist-in-Chief at Johns Hopkins Hospital, with secondary appointments as Professor of Oncology, Neurosurgery and Biomedical Engineering.

Prior to his position at Johns Hopkins, Dr. Lewin was the Director of the Division of Magnetic Resonance Imaging at the University Hospitals of Cleveland and Vice Chairman for Research and Academic Affairs in the Department of Radiology at Case Western Reserve University.

Dr. Lewin received his undergraduate degree in chemistry from Brown University in 1981 and his Doctor of Medicine from Yale University in 1985. Following his internship in pediatrics at Yale-New Haven Hospital and residency in diagnostic radiology at University Hospitals of Cleveland, he completed a magnetic resonance research fellowship in Germany, a neuroradiology fellowship at the Cleveland Clinic, and additional training in head and neck radiology at the Pittsburgh Eye and Ear Hospital.

Dr. Lewin has been a pioneer in interventional and intraoperative MR imaging and has published approximately 200 peer-reviewed scientific manuscripts and more than 60 chapters, reviews, commentaries and other invited papers on topics, including the basic science and clinical aspects of interventional MR imaging, functional MR imaging, head and neck imaging, MR angiography, small animal imaging and the imaging of acute stroke.

Dr. Lewin is an inventor on more than 25 patents, and has been PI and co-PI on NIH and other federal and state grants with awards of more than $10 million in direct costs, as well as a co-investigator on a number of other grants and projects. He is a Fellow of the International Society for Magnetic Resonance in Medicine and of the American College of Radiology, and has given more than 150 invited lectures on a number of topics in magnetic resonance imaging, intervention and neuroradiology. He has served on numerous national committees, editorial boards and grant review groups for foundations and the NIH, and on the task force on minimally invasive cancer therapy for the National Cancer Institute.

He is currently president of the American Roentgen Ray Society (North America's oldest Radiology professional organization), president of the International Society for Strategic Studies in Radiology, president-elect of the Society of Chairs of Academic Radiology Departments, and immediate past-president of the Association of University Radiologists and of the Academy for Radiology Research.

Ms. Mathis serves as the Associate Administrator of Population Health and Disease Management within the Ambulatory Care Services department. The Ambulatory Care Services department provides more than one million primary care visits each year to patients. Today, it is the largest public primary care health network in the state of Texas offering specialty and acute care at three hospitals and primary care at twenty-three community health centers. Ms. Mathis has oversight over Ambulatory Clinical Case Management, Patient Education, Nutrition Services and Community Outreach departments. She oversees employees who are located in the three hospitals and the twenty-three community health centers. Ms. Mathis has operational and clinical oversight with nurses, dieticians, patient educators, social workers and clinical case managers establishing her as a system leader within population health. Currently, her focus area is to define and implement the population health strategy for the organization.

Ms. Mathis has oversight of nutrition, case management and patient education, and is leading a population health initiative to decrease the hemoglobin A1c (HbA1c) by facilitating a pilot using a multi-disciplinary approach to patient care. This initiative allows patients the opportunity to receive same day interventions for their needs rather than scheduling return clinic appointments (i.e. diabetes class, healthy eating class, etc.). The pilot focuses on identifying the patient’s greatest need and any barriers to care the patient may have on the day of their primary care visit. After the assessment is completed upon their primary care visit discharge, the multi-disciplinary team follows up with the patient with several interventions based off of the care plan developed by the team. The multi-disciplinary team is made up of the primary care physician, nursing, nutrition, clinical case management, social work, patient educator and clinical pharmacist. This pilot has shown an average of a 3% reduction in HbA1c and is being reviewed to be used by all community health centers in our system as a practice model of how to care for diabetes patients who are uncontrolled or newly diagnosed.

Ms. Mathis has also been the co-facilitator in a readmission reduction plan for the system which involves electronic transition of care referrals being sent from the inpatient case managers to the ambulatory case managers. The goal of the transition of care referral was to develop a consistent process for transitioning high risk patients from an acute care setting to the ambulatory care setting which would ensure services, follow up care and support are available. The development of this referral has resulted in an improvement of workflow, data collection, reporting, and the monitoring of patients’ utilization of services. Currently, our system has seen a decrease in 30 day readmissions and patients are having a better experience when being transitioned from inpatient to the community.

Ms. Mathis assists nursing and clinical staff in expanding their nursing and leadership knowledge in various ways. She is a preceptor to Master of Science in nursing students along with being a mentor to nurses within our healthcare system. She loves to teach and is always available. She is always being asked to participate in system wide initiatives, is on multiple committees throughout our healthcare system and involved in several initiatives as a community partner.

Akil McClay is a System Director of APM Operations at Trinity Health. In this role, Akil is responsible for the implementation, deployment and operational CIN/ACO/APM activities across 7 states (Michigan, Delaware, Massachusetts, Maryland, Pennsylvania, Connecticut and New York) with approximately 500,000 covered lives. Additionally, Akil serves as a lead for the Trinity Health Integrated Care MSSP Track 3 ACO. In Akil's prior role, he served as the System Director of Value?Based Care & Payment for SSM Health. Akil led operational oversight and implementation of population health initiatives across 4 states (Missouri, Illinois, Oklahoma and Wisconsin) and served as the lead for ACO and CIN operations. Some of Akil's experience includes the successful development and deployment of employee onsite clinics, primary care telehealth, ACO operations and CIN payer contract negotiations.

Prior to joining SSM Health, Akil served as the Administrative Officer of Research & Development for the VA St. Louis Healthcare System. In this role, Akil managed the daily operations of the Research & Development Department and Clinical Epidemiology Center. Akil was responsible for the budget, department operations, management of human research studies, animal protocols, and basic science studies as well as departmental standard operating procedures and personnel contracts. Akil also served the VA St. Louis Healthcare System in Extended Care and Rehabilitation Service, Budget Analytics, and Executive Office roles.

Akil holds a B.S. in Psychology from Southern University, a M.S. in Neuroscience from Tulane University, and a M.H.A from the School of Health Management and Informatics at the University of Missouri.

Dr. Mims is board certified in family medicine and geriatric medicine, with fellowship status in both specialties. She has clinical practice experience in caring for Medicare beneficiaries including hospital, office, nursing home, hospice and home care. As a clinical epidemiologist, she has with more than 20 years of administrative experience in using quality performance metrics to guide changes in clinical processes, clinician and patient education and organizational delivery system redesign in small clinical practices, large disease management, Medicare Advantage and managed care organizations.

Dr. Mims serves and the Vice President, Chief Medical Officer for Alliant Quality the CMS contracted Quality Innovation Network – Quality Improvement Organization for North Carolina and Georgia.

Brad Miner, Sr. National Director of Client Services and Clinical Programming for Rehabcare. Brad has spent his entire career in healthcare. He has worked within operations, sales and business development. He has worked with LTAC and Inpatient Rehab Hospitals previous to joining Kindred Healthcare and Rehabcare. He is currently focused on partnering with Rehabcare Hospital, Skilled and Home Health clients on physician relations, preferred provider, ACO, Bundling and Program Development opportunities across the country.

Rachele Misiti, LCSW is a Supervisory social worker at Central Texas VA Healthcare System. After starting her career in community care she transitioned to the VA in the Transition and Care Management program as a case manager in 2007. After years of providing case management to Seriously injured combat Veterans, she transitioned to providing leadership the Transition and Care Management Program. Additionally, Ms. Misiti provides oversight to the VA/DOD Liaison Program at Ft Hood and Primary Care Social Work. Central Texas TCM teams were recipients of the 2015 Transition and Care Management Team of the Year from the VA Care Management Social Work Service. Rachele Misiti leads CTX Social Work Service’s Quality Value and Safety Committee. Ms. Misiti is an avid Dressage horseback rider and fiber artist.

Jennifer Montgomery, is the Director of Physician Revenue Enhancement at Mount Sinai Health System in New York City. Along with her team, Jennifer is responsible for the Meaningful Use/PQRS Programs for Mount Sinai’s integrated health care practices and now plays an integral role in transforming these programs into the new MACRA-QPP program. Jennifer has an extensive, varied and comprehensive background in both Medical Assisting and Medical Practice Administration working for several different specialties for more than 30 years. She became certified as a medical manager and served as President of the Nassau County Chapter of the Professional Association of Healthcare Offices Managers (PAHCOM), was instrumental in forming the Nassau County Chapter of PAHCOM and served as a consultant for a few years prior to joining the Faculty Practice Central Billing Office at Continuum Health Partners in 2002. During her employment at Continuum, Jennifer worked to help improve revenue in various ways including working with practices to help educate staff and improve workflows; assist IT in implementing and understanding the HIPAA SDE updates, and was assigned to a new team to assist in implementing new software technology meant to enhance and streamline provider charge capture that would produce “clean claims”, reduce charge lag and increase revenue. She was also instrumental in impacting the charge capture software to implement a way to capture PQRS quality measures during the patient encounter for submission of the PQRS quality measures. During her time at Continuum Jennifer taught Medical Administrative classes at Molloy College and became certified as a procedural coder in 2009. In 2011 Jennifer was asked to take part in the Meaningful Use Workgroup being formed at Continuum and became responsible for the administrative registration/submission and payment aspects of the program. She worked closely with clinical and senior leadership to understand the incentive policies, provide risk analysis and strategies to ensure compliance and revenue. Continuum merged with Mount Sinai in 2013. Jennifer’s team has continued to work with practices to understand and comply with PQRS, Meaningful Use programs and they have worked with different departments to ensure and implement policies and methods to ensure a smooth transition into value based payments and programs. Merging systems, departments and programs has been ongoing and present many challenges and currently they are working to ensure a successful transition into the MACRA-MIPS arena along with a very robust and talented group comprised of senior leadership, administrators and providers. “I’ve been fortunate to be part of an incredible group of professionals, comprised of our senior and executive leadership and a workgroup of dedicated and talented providers and administrative personnel in preparing and positioning our very large healthcare system to successfully navigate a successful transition into this new phase of healthcare”.

Mr. Mueller is the Manager of Healthcare Quality for the American Medical Association (AMA). In this role, he supports the AMA on a multi-year journey identifying effective care delivery elements that support professional satisfaction and practice sustainability. He manages AMA’s efforts to support physician practices, including such efforts as the Support and Alignment Network (SAN) and the creation of physician practice improvement capabilities that can be deployed on the ground at practice sites. Many of these tools are focused on changes related to the Quality Payment Program.

Prior to joining the AMA, Mr. Mueller was the Director of the Anesthesia Quality Institute (AQI). AQI hosts a variety of clinical data registries including the National Anesthesia Clinical Outcomes Registry (NACOR) and the Anesthesia Incident Reporting System (AIRS) – one of the first nationwide anesthesia safety reporting systems. During his time at AQI, NACOR grew from 4 practices/250K cases to 400+ practices with over 32M cases. AQI was also in the inaugural class of Qualified Clinical Registries (QCDRs) beginning in the 2014 program year. Mr. Mueller helped develop MIPS measures for the AQI QCDR. AQI successfully reported on behalf of thousands of anesthesia providers via QCDR for program year 2015.

Mr. Mueller completed his formal education at Northern Illinois University (NIU) attaining a Master’s of Science in Mathematics. He worked in the IT space prior to joining the registry and association world.

Lee Pearce started his healthcare career as a social worker after completing his masters in social work from the University of Tennessee in 1996. He worked primarily in the home health environment, but also spent time as the Director of Discharge Planning in an acute care hospital. In 2000, Mr. Pearce went back to school and received his masters in healthcare administration in 2002 where he subsequently went into administration with the Department of Veterans Affairs. His responsibilities over the next decade primarily encompassed oversight of the state’s Community Based Outpatient Clinics and the Department of Surgery. In 2011, Mr. Pearce accepted the position of Vice President at the Alabama Quality Assurance Foundation. Since this time, his primary role has been the management of contracts with the Centers for Medicare and Medicaid Services. He also obtained his Fellow status with the American College of Healthcare Executives in 2014.

Sherrie Petersen is the director of LivingWell@Home and has been a part of this emerging service for the past four years. Her background includes 20 years in healthcare management and she has worked in all aspects of healthcare delivery. She has an undergraduate degree in social work/psychology and a graduate degree in business administration with management emphasis. She continues to strive to enhance health outcomes of those with chronic health conditions through remote patient monitoring.

Dorothy A. Sanders, BSN, RN is the Nurse Manager, Integrated Case Management & Home Telehealth Program at Central Texas Veterans Health Care System in Temple, Texas. Ms. Sanders has chosen to serve Veterans at the VA for her entire nursing career to honor our Nation’s heroes. She began her nursing career with the VA shortly upon graduating from University of Mary-Hardin Baylor in June 2004, where she cared for Veterans admitted on the Acute Medicine/Telemetry inpatient ward. In 2008, she became a Patient Care Coordinator, learning case management strategies and approaches, while coordinating inpatient care for the Veteran admitted to Acute Care and guiding their transition to home upon discharge. In this role, she led a system redesign project implementing daily interdisciplinary team meetings which resulted in a significant decrease in the average length of stay from 8.29 days to 5.54 days. In 2010, she served as the facility’s Performance Measures Coordinator and Executive Assistant to the Director, developing her systematic and organizational thinking. Missing her passion for Nursing and Case Management, she accepted the position as Nurse Manager for Case Management in November 2012. She has been instrumental in the expansion and further development of the Nursing Case Management Program, including the realignment of the Home Telehealth Program to reduce duplication in nursing case management efforts. On a personal note, Ms. Sanders is a mother of two boys and wife to a loving husband, who keep her youthful in her hobbies and fun time. She is an avid Star Wars fan, and has frequent lightsaber duels on family night with her boys.

With more than 13 years of experience with the Department of Veterans Affairs, Mr. Christopher Sandles joined Central Texas in March 2017 from the Michael E. DeBakey VA Medical Center (MEDVAMC) in Houston, Texas, where he served as Associate Director/Chief Operating Officer since April 2014. As MEDVAMC's Chief Operating Officer Mr. Sandles was responsible for an operating budget of $976 million, and direct oversight responsibility for Finance, Human Resources, Health Administration Service, VA Care in the Community, Logistics, Pharmacy, Clinical Documentation Improvement, Contracting as well as Clinical Application Coordinators. His years of VA service include positions as Assistant Director of VA Greater Los Angeles Healthcare System, Chief of Health Administration Service at VA Loma Linda Health Care System, Assistant Chief of Medical Administration Service at VA North Texas Health Care System, Special Assistant to the Director-VA North Texas Health Care System, and Administrative Director for Pathology and Laboratory Medicine Service. Mr. Sandles began his healthcare career in 2002 at Covenant Health System in Lubbock, Texas. In 2003, he was accepted into the Government Health Administration Training Program (GHATP) and served his post graduate fellowship with VA North Texas Healthcare System. Mr. Sandles received both his Bachelor's degree in Business and his Masters of Healthcare Administration from Texas Tech University where he served as President of the Forum Chapter of Mortar Board and was a sitting member of national council. Mr. Sandles also holds a green belt certification in six-sigma from Villanova University and is a Fellow in the American College of Healthcare Executives. Mr. Sandles was recently accepted into the United States Navy Reserve and will be commissioned as a Lieutenant. Central Texas Veterans Health Care System (CTVHCS) consists of two hospitals located in Temple and Waco; one large outpatient clinic located in Austin; four community based outpatient clinics (CBOCs); and operates a total of 632 beds (154 inpatient, 200 Community Living Center, and 278 Domiciliary Residential Rehabilitation Treatment Program beds). Mr. Sandles has an exceptional background and possesses the leadership, knowledge, and skills necessary to successfully lead the Central Texas Veterans Health Care System into the future.

Mary Simpson is a Health Information Technology Specialist for the Quality Payment Program at Alliant Quality, and provides technical assistance to large and small practices. Before joining Alliant Quality, Mary served as Manager of Training for an electronic health record implementation at a large hospital in Savannah, GA.. She has over 25 years experience in healthcare, including practice management as well as technical experience with multiple EHR and practice management systems.

Bryan W. Sisk, RN, BSN, MPH is the Nurse Executive/Associate Director for Patient Care Services for Central Texas Veterans Health Care System. In this capacity, he is responsible for all nursing, social work, sterile processing, and infection prevention services. Mr. Sisk formerly served as the Deputy Nurse Executive and Associate Chief Nurse for Ambulatory Care at CTVHCS in Temple, Texas. Mr. Sisk entered the Department of Veterans Affairs in 2008. Prior to that, he spent more than 20 years in the Army where he held multiple leadership, academic and clinical roles. Mr. Sisk holds a Master of Public Health from Texas A&M; University’s School of Rural Public Health, as well as a Bachelor’s of Science degree in nursing from Arkansas Tech University in Russellville, Arkansas. He has multiple publications, maintains an Adjunct faculty role at Texas A&M; University’s School of Rural Public Health, and has been a champion for care coordination and case management in his roles in both the DoD and VHA.

Dr. Karen Smith is independent rural Family Physician with twenty-six years of practice in Raeford, North Carolina. Her PCMH recognized practice acknowledges success due to 2003 early adoption of electronic health records and dedicated workflow which addresses the needs of the community. Through her advocacy work with both North Carolina and American Academy of Family Physicians she successfully communicates real issues with provision of care in rural medicine. Dr. Smith welcomes opportunities to learn, share, and express thoughts which foster improved health in our society.

JANET TOMCAVAGE, RN, MSN, is Chief Clinical Transformation Officer for Geisinger Health Plan in Danville, Pennsylvania. In this role, she is responsible for the administrative oversight of health services including medical management, appeals and grievances, quality improvement, clinical reporting, accreditation, medical home, disease/case management, and wellness.

Ms. Tomcavage has worked for over 20 years in the specialty of diabetes management, developing a comprehensive diabetes self-management education program and specialized care management programs for insulin pump therapy and high-risk pregnancy. More recently her work has focused on the development and implementation of enhanced medical management strategies including medical home, disease management and case management interventions that optimize quality and efficiency outcomes.

Ms. Tomcavage earned her bachelor of science in nursing from Bloomsburg State University and her master of science in nursing from College Misericordia. Ms. Tomcavage has held certifications as a certified diabetes educator and advanced practice for diabetes management. She has co-authored several articles on diabetes management and disease management and has lectured nationally on diabetes management, disease and case management, medical home, and other medical management strategies.

A serial entrepreneur, and board-certified radiologist is the CEO of NucleusHealth, an active leader in cloud medical data management and interpretation of complex medical images to speed diagnosis, and treatment planning. Under the leadership of Dr. Verma, NucleusHealth has grown nearly 5X since 2010 and now currently provides service to over 400 facilities for teleradiology and over 2400 facilities for cloud medical image management. Dr. Verma’s focus has been empowering patients with greater access and control of their healthcare journey by leveraging novel technologies like blockchain. Dr. Verma’s extensive background in radiology combined with a passion for better workflows and solutions to improve patient care while lowering costs all started with a medical degree from the University of Miami. Dr. Verma served as a Chief Resident at the University of Chicago and then completed an MRI fellowship at the University of California San Diego. He is currently licensed to practice in all 50 states. Dr. Verma has a strong track record of entrepreneurship and enjoys the challenge of bringing new ideas to market.

Betsey Walton is the Executive Director of United Health Alliance. Since 2015 she has overseen credentialing and contracting for the 100+ members of the Physician Hospital Organization. In this position she provides support and education to members and their staff on the changes and regulations happening in healthcare. She works with member practices on the various reporting requirements of the Accountable Care Organization, the Blueprint for Health, National Committee for Quality Assurance Patient Centered Medical Home, Meaningful Use, Merit Based Incentive Payment System and various other payer initiatives. Her 20 years’ experience in practice management in both independent and health system owned practice environments has allowed her to understand the nuances of implementing change in different situations. She worked for Vermont’s Regional Extension Center creating successful tools for the implementation of Electronic Health Record systems. Throughout the state she educated medical practice teams to help their practices meet the requirements of Meaningful Use. She is HIPAA certified and has performed many Security Risk Assessments for practices attesting to Meaningful Use and other programs. She collaborates with multiple organizations and in-house departments to bring cohesiveness to the MIPS initiative, helping to align measures and goals in this ever changing environment.

Ms. Walton is a graduate of Boston University, School of Management with a degree in Business Administration. She serves on the Advisory Board for the local free health clinic and the Board of Directors for Rutland Rocks Curling Club.

Beth Weagraff serves as AHS’s vice president of post-acute care development and implementation. In this capacity, she is responsible for the development of the AHS Post Acute strategies and ensuring successful deployment of post acute initiatives. Prior to this position, she served as the COO for Florida Hospital Medical Group and served as senior vice president for Florida Hospital, where she provided oversight for the Cardiovascular Institute, Neuroscience Institute and the Institute for Surgical Advancement. She began her services with Adventist Heath System as a registered nurse for more than 30 years.

Weagraff has both a bachelor’s in nursing and a master’s in business administration. Beth has authored articles on Business Process Redesign while working in performance improvement areas of Florida Hospital. She is active in her church and community, having served as Chair for the American Heart Association’s Heart Ball and Go Red for Women Campaign in Central Florida. She has also served on the Board of Directors for FH Memorial Medical Center.

Sherry Westlake is the Vice President of Sales for Scottsdale-based company, HealthBI. With 20+ years of subject matter expertise in population health, value-based care strategy, product management, and health care analytics. She is responsible for both strategic and technical solution consulting and sales. In her current and previous roles, with McKesson Technologies, MedeAnalytics, NantHealth, and now HealthBI she is assists in developing enterprise population health solution strategies and new business partner relationships. In addition to her role in business development, Sherry continues to provide thought leadership on healthcare integration, integrated care delivery models and has contributed thought leadership content for Healthcare IT Analytics Media, Georgia Council for Provider Integration and as a national thought leader speaker for HIMSS.

Dr. Wild received his MD degree from the University of Vermont, College of Medicine. He is board certified in Emergency Medicine by the American Board of Emergency Medicine and is a Fellow of the American College of Emergency Physicians. His internship and residency training were at Letterman Army Medical Center, San Francisco; San Francisco General Hospital; and Walter Reed Army Medical Center in Washington, DC. He served five years as an active duty officer with the US Army Medical Corps and also served as a Department of Defense civilian emergency medicine specialist at Martin Army Hospital, Fort Benning, Georgia. Dr Wild earned his BA in Biology from Dartmouth College, his JD degree from the George Washington University National Law Center in Washington, DC, and his MBA from the Columbia University Graduate Business School, in New York City.

Dr. Wild has 30 years of experience as a practicing emergency physician ranging from small rural Georgia hospitals to large university teaching centers. He has served on the Medical School faculties of Boston University, University of Massachusetts, and Brown University and was Chairman of the department of Emergency Medicine and Chief of Emergency Services at a teaching hospital of Brown University Medical School. He has taught emergency medicine, health policy and legal medicine to resident physicians and medical students and has served as President of the Rhode Island College of Emergency Physicians, and as chairman or member of several American College of Emergency Physicians (ACEP) and American Medical Association (AMA) national committees dealing with government affairs, coding, and physician reimbursement.

Dr. Wild is an active member of the Massachusetts and US Federal bar and has practiced as a health care attorney with a large Boston law firm representing hospitals, physicians, skilled nursing facilities and a major Boston teaching hospital. He was medical director of Medicare’s direct fiscal intermediary in Baltimore and also CMS (then HCFA) Chief Medical Officer for reimbursement policy during the initial implementation the Hospital Prospective Payment (DRG) system. He subsequently served on the Medicare Prospective Payment Assessment Commission staff (now MedPac).
