| W. Michael Hooten, M.D., graduated from Saint Louis University School of Medicine in 1992. He subsequently pursued residency training in psychiatry and internal medicine at Johns Hopkins School of Medicine, Yale University School of Medicine and Mayo Graduate School of Medicine in Rochester, Minnesota. During two years of community-based practice, he developed an interest in pain medicine and pursued further residency training in anesthesiology at Washington University School of Medicine in St. Louis, Missouri. Dr. Hooten completed a pain medicine fellowship at Mayo Graduate School of Medicine and is currently a staff physician at Mayo Clinic in Rochester, Minnesota. He has a joint appointment in the Department of Anesthesiology and Department of Psychiatry and Psychology and is the associate medical director of the Mayo Pain Rehabilitation Center. He is currently board certified in anesthesiology, psychiatry and internal medicine. |

| Christopher Sletten, M.D., graduated from Louisiana State University in 1995. He subsequently pursued internship training in clinical psychology at the University of Mississippi Medical Center/ Jackson VA, in Jackson, Mississippi. He completed his training during a two-year post-doctoral fellowship at the Mayo Clinic, Rochester, Minnesota. Dr. Sletten joined the staff of the Mayo Clinic in 1996. Since then, he has served as associate clinical director of the Mayo Comprehensive Pain Rehabilitation Center. |

| Aubrey Swartz, M.D., provides comprehensive care to workers’ compensation injuries and to orthopedic patients with musculoskeletal conditions. He lectures on workers’ compensation injurie and disability evaluations to health care providers. He is a Qualified Medical Examiner for the States of California and Nevada and is a Certified Independent Medical Examiner, as well as a Medical Expert for the Social Security Administration Office of Hearings and Appeals, and a Medical Consultant for the Medical Board of California and the U.S. Department of Defense. As Executive Director of the American Back Society, Dr. Swartz edits the American Back Society Newsletter. He is a Fellow of the American Academy of Orthopedic Surgeons, North American Spine Fellow, American Orthopedic Foot and Ankle Society and International College of Surgeons. |
