Objective: Examine the provider/patient relationship to build a platform for timely,
effective communication for improved patient engagement and behavior change
The value of health is determined by quality indicators divided by total costs to
the organization. Health improvement can be driven by aligning the incentives
with clinicians and payers to drive better outcomes. The patient center
collaborative model is getting national attention as a management model that
rewards physicians and caregivers for improved patient health and reduced
preventable inpatient days. This workshop will provide the evidence that the
collaborative care model works, and those physicians, patients, and payers all
succeed when the incentives are aligned.
