“A Model for Integrating Independent Physicians into ACOs”!

A Model for Integrating Independent Physicians into ACOs

  • Discuss how Advocate Physician Partners overcame challenges to widespread adoption of ACOs – dominance of small physician practices, traditional hospital medical staff structures, fee-for-service reimbursement and private-sector acceptance – with its structures and processes
  • Examine why this model has broad applicability for the future of ACOs and identify what the hurdles are to adapting this model elsewhere
  • Hear from a medical practice leader from a Midwest IPA on the challenges of implementing their medical home model

Under the Patient Protection and Affordable Care Act (PPACA) of 2010, Centers for Medicare & Medicaid Services (CMS) is mandated to establish a new Center for Medicare and Medicaid Innovation (CMI) in 2011. The mission of CMI is to develop innovative ways of providing and paying for health care that have the potential to reduce costs while preserving or enhancing health care quality.

In the spirit of that new mandate, the World Congress will offer The World Congress 2nd Annual Leadership Summit on Achieving the Triple Aim* Objectives of Health Care. The objective is to provide a forum where providers and payers can interact and share ideas on how to achieve a high performance health system thus bridging the gaps between organizations. Real-life examples of delivery and payment models will be discussed and the audience will be provided with insight on the promises and pitfalls of innovative programs that pay for coordinated care from the hospital, physician, payer and employer perspectives.

By attending this Summit you will learn how to:
  • SET standards for performance and quality reporting to achieve patient-centered care
  • DEVELOP a community-based, collaborative approach to transforming the health care system
  • ASSESS the technology and tools needed to answer questions on clinical care in real-time to develop outreach strategies for improvement efforts
  • EVALUATE payment models and learn how they can be made to work in the reform environment
  • CREATE alignment structures for gain-sharing, bundled payments and ACOs
  • DESIGN and implement incentives that effectively reward efficiency and quality of care delivery
  • INTEGRATE and pay for specialist and hospital care in a primary care delivery model
  • UNCOVER opportunities for improvement when variation is not explained by the patient population through systematic analyses of data
