Health care organizations face an ever-growing list of challenges – poor patient outcomes, rising costs, inefficient systems, compromised quality of care and evolving
health care technology. The spotlight is shining brighter than ever on transforming America’s health care system; a transformation that must be deeply embedded with
patient safety and high quality care. Two emerging models of care offer promising changes to improve both the coordination and quality of patient’s care – the
Accountable Care Organization (ACO) and the Patient-Centered Medical Home (PCMH).
Changes of this degree require system-wide interventions to improve processes within and across entire health care organizations. This interactive, hands-on workshop
will introduce participants to a quality improvement methodology, known as Perfecting Patient CareSM, rooted in Lean concepts and validated through 10 years of direct
health care application. Participants will have the opportunity to learn and apply proven Lean tools and principles that will enable staff at all levels of these organizations
(frontline, providers, management, etc.) to become problem-solvers and begin addressing the challenges in their own organizations, from communication within a unit or
practice to communication between systems and beyond. The workshop will focus on improving the quality of care for patients who transition along the health care
continuum, an essential element of the ACO model, and how the PCMH serves as a foundation for quality patient care delivery
Following participation in this workshop, you will be able to:
- Discuss how the needs of the patient will guide improvement efforts.
- Describe how lean tools and concepts can contribute to the successful formation and sustainment of an ACO or PCMH.
- Recognize the benefits of constructing value stream maps as a means to understand patient care and to work towards sustainable care models.
- Articulate methods to redesign health care delivery systems around the ACO and PCMH models with a focus on team-based care coordination and communication.
